The personal data collected includes; full name, email address, phone number, date of birth, gender, profile picture, cancer type, banking information, professional license numbers, medical records and other information that identify you (collectively referred to as “Personal Information” of “Personal Data”).
You are also required to create a login profile for your Oncopadi account with a username and password, as generated by you.
By providing your Personal Information to us, you expressly agree to our collection, use, storage, and disclosure of such information as described in this Privacy Policy and in accordance with the extant Data Protection Laws of the Federal Republic of Nigeria.

Such independent Arbitrator shall be charged with the responsibility of resolving the said dispute and the Arbitrator’s fees shall be borne equally by both parties.
